FOR APPLICANTS WITHOUT EXISTING CREDIT CARD/S

Photocopy of the following:

1. PROOF OF INCOME
1. For EMPLOYED (Required):
1. Photocopy of latest BIR Form 2316 signed by employer’s authorized representative AND
2. Submit ANY ONE of the following (Optional):
• Original Copy of Certificate of Employment and Income (COEI) indicating status, length of service and compensation breakdown
• Photocopy of latest full month payslip.
2. For SELF-EMPLOYED:
1. Registration of Business Name (Required):
2. Photocopy of latest ITR and W4 with BIR/Bank stamp (Required);
3. Submit ANY ONE of the following (Required):
• Audited financial statements for the last 2 years; OR
• Bank statements for the last 3 months
2. PROOF OF IDENTIFICATION
   Front and back portion of a valid ID with photo and signature such as: Company ID; TIN/SSS/GSIS/Pag-ibig or other government-issued ID; Philippine Passport; Driver’s License; PRC ID.
